I certify that I am initiating a consulting relationship with Martha M. Faraday and Four Oaks for the provision of an equine nutritional evaluation and recommendations to optimize my horse’s health. I understand that Martha M. Faraday and Four Oaks are not veterinarians, do not provide veterinary services, and that information derived from this consultation is not a substitute for veterinary services.

I hereby completely release Martha M. Faraday and Four Oaks and their agents, employees, assistants, independent contractors, cosponsors, and affiliates, of any liability related to the provision of consultative services.

I acknowledge that I have been given the opportunity to ask questions regarding any aspect of this release, and by signing below, I acknowledge that I fully understand all aspects of the release and agree to these terms in their entirety. I hereby agree that should any legal proceeding arise relating to this general release and agreement, the disallowance of any term or condition will not prevent the legal enforcement of all other terms and conditions of this agreement.

I execute this release with the express intention of effecting the legal consequences, that is, the release of all liability associated with participating in equine consultative activities provided by Martha M. Faraday and Four Oaks, for myself, my executors, administrators, heirs, next of kin, successors, and assigns. This agreement shall constitute the full and complete understanding between the parties and shall supersede any prior or contemporaneous representations or agreements, oral or written. This agreement shall be interpreted pursuant to and governed by the laws of the State of Virginia. If any action at law or equity is brought to enforce any of the terms of this agreement, the prevailing party shall be entitled to actual attorney's fees and collection fees. It is agreed that the venue for any such action at law is Clarke County, VA. No assignment of this agreement is permitted and any attempt to assign all or any rights, obligations or duties granted under this agreement shall be null and void.
